I swear I followed the guide including nuclear option and normal paragraph style but the blanks and un-bold paragraphs tell a different story. Any ideas how to correct this? I've done the space bar check and didn't use the heading styles.

Blanks - you probably have too many empty lines that you can't see. Turn on all formatting marks so you can see what's what. In Word 2010 it's under File / Options / Display/ all formatting marks. Previous versions go to Tools/Options. Yes it might look untidy but at least you can see your paragraph end markers and spaces etc.
Not sure what you mean about non-bold. Doing the nuclear is supposed to strip all formatting off the document so you can start again, afresh.
Perhaps your document has corrupted, too. You can never tell what Word is likely to do next. Maybe select all, (ctrl+a) copy/paste special/unformatted text into a brand new document? If you do this, make sure you paste in on a line that has NORMAL style applied! (Don't do a simple copy paste. Doing that brings in the gremlins you're trying to get rid of.) After that you have to reapply italics/bold, chapter headings etc.

All formatting marks already on. Blank pages only in EPUB. Mobi looks okay.
Since realised the non-bold paragraphs all contain one or two words in italics. So un-italicised italicised words. Paragraphs look okay. Problem solved.

Four returns will automatically generate a new page so if eight or more are present, you'll get a blank page. You can also set Word doc/docx files to start new chapters on odd numbered (right hand) pages. This would generate a blank page at chapter ends sometimes. I always use heading 1 style for chapter headings - it makes creating a TOC so much simpler. Italics don't usually cause a problem other than when making a printed book. CreateSpace & Lulu will tell you how much space you MUST leave in gutter, inner and outer margins. If you are using these values you'll find italics can creep into these margins causing the book to be rejected. I always add an extra 1mm to the inside/gutter margins to allow for this.
