Can I do this on the front matter publisher line:
Published by Steve Frank (Dented Cranium Publishing) at Smashwords
Adding the "(Dented Cranium Publishing)" is allowed?

I'd go with what is suggested in the Style Guide.

Can't say as I've never done it that way. If rejected then it's easily fixed.
