My book was rejected from the premium catalog saying "Only two items appear in the NCX". Help! I don't know what this means Thanks!

I think it means you need more chapters.

Amber, review your Bookmarks and Hyperlinks for your chapters and make sure they are correct. Reread the Style Guide for more information on Bookmarks and Hyperlinks.

NCX is part of the ePub format. It is your Table Of Contents. If you didn't create your own TOC with links to the chapters or sections of your work, then ePub tries to create them for you.
Most ebooks have a TOC, with links to different sections of the ebook making it easy for readers to move through the book.
I find it is always best to manually create your own TOC and hyperlinks. Follow the Style Guide and you shouldn't have any problems.

Amber, unlike Ted I don't create my own TOC, but haven't had any issues with NCX being automatically created in my books. I have each chapter simply titled Chapter x, and the text is in the normal style (ie that recommended in the Style Guide), but bolded and centred - not created using Headings in MS Word. NCX seems to be able to recognise that as a link for a TOC and automatically creates the TOC and the correct links in the book.
I hope that didn't sound too convoluted :-)

I have a question:
My book has been awaiting approval on Smashwords since April 1st. They never told me of errors to correct or anything.
Is this a normal waiting time? Thanks.

Yes, depending upon the number of other books ahead of yours. Look on your Dashboard, in the green colored column, where it says "Awaiting Review", and click on the "(details)" hyperlink for information on your book.

Also study the Smashwords Distribution Information page. We make the requirements VERY clear because we want you in the catalog! ***** It worries me because I've done all they said. My first book went through without a hitch and it was in the exact same formatting as this one. It says it may never be reviewed if there are errors found, but I haven't been told of any. Also, I have no way of seeing how many people are ahead of me. Is there a way to see this?

I wouldn't worry. Your first book went through fine, and you say your second is formatted exactly the same. The delay may simply be caused by a very high number of books being published on Smashwords recently.
The thing I like about Smashwords compared with Amazon is Smashwords actually reviews your book before it goes to Premium Distribution. According to a Kindleboards, Amazon just lets buyers of their Kindle ebooks be the editors in that Kindle ebook purchasers write and complain to Amazon about spelling errors, grammatical errors, and other formatting issues in KDP ebooks.
