I submitted a new cover image for one of my books. Smashwords has this option to upload a new cover. Conversion complete; approved for Premium Catalog.
But ... when I download the ePub, the new cover is on the outside, but the old cover is the first page on the inside. Do I have to REPUBLISH the book to remove this image? If so, what was the point of having the option to only upload a cover image? I'm not sure what to do now.

That is weird. The cover 'should' update automatically in all formats. But then we are talking computers and they aren't perfect. Download a PDF, mobi, and the ePub again. The ePub should display your latest cover, and not include your previous cover.
I've never had a cover included in a PDF. On my Kindle for PC, a work opens 'without' a cover, but if you go back one page the cover appears. On ePub works the cover appears only once, before the front matter.
If your new cover and old cover appears only in ePub then you have a choice to 1) notify Smashwords of the problem and ask them to look into it and correct it, or 2) upload your work again.
If you choose 2 then send support a note telling them why you uploaded a new work, ask them to have tech people look into why old cover wasn't updated in ePub, and ask if they can rush Premium approval as you only uploaded your work again in the hope of correcting ePub problem.

Ted, do you think it will take some time before the changes are made? The cover change was made on Monday, and approved for premium catalog yesterday. The new cover shows on the page view, and at the beginning of the epub, but the second page of the epub is the old cover, and the old cover still shows for all other formats - no new cover at all.
Shall I wait a few more days to see if the changes show?

Check your 'Details' link for your work on your Dashboard and see if cover is awaiting review. I've had covers take a few days to fully update as Smashwords manually reviews cover changes for Premium distribution to ensure new cover complies with guidelines.
