I don't know if anyone has no trouble getting directions but I have none. I posted a question once before and got some helpful answers at Smashwords forums. Then I was accepted into the Premium Catalog. I also got a tip from Smashwords customer service to buy a book on formatting with OpenOffice. I then realized I had already been accepted into the Smashwords Premium Catalog. The point is I found out my book does not have a separate copyright page and title page as specified in Smashwords Guidelines in the Openoffice book. I feel I may have been accepted outside of Smashwords rules, and that's why I have made no sales? My book somehow looks as if it is dead, despite being in the Smashwords Premium Catalog.
Please can people comment.

You should look at sample pages from other authors to see how they layout/format their copyright page and how they flow their words on the pages.
Here are some example front matter (copyright, author, etc.) from the Smashwords FAQ's. Follow these examples for a proper front matter:
Book Title
FirstName LastName
Copyright 2010 by FirstName LastName
Smashwords Edition
****
Or, this is a good short one too
****
Book Title
FirstName LastName
Copyright FirstName LastName 2010
Published at Smashwords
****
Or, if you're a publisher, this is a good one
