I have been having continual rejections prior to being admitted into the premium catalog. I created an epub book from my text using Calibre to see if I could find the source of my errors. All hyperlinks in Calibre default to the bottom of the page. I have also not been able to access an index or ndx file because I cannot find the ndx file. Can someone please help with these issues.

It's your booksmarks that are the problem. Follow these steps. First select the text to which the hyperlink should lead to. Then do insert > bookmark You'll get a screen like this: Give your bookmark a name. This can be one word or a phrase, as long as you can recognize it later on. (Mind: don't use spaces. Use underscores instead). In this example I've given it the name 'chapter_1' but it could be 'Adversity' too. After you've typed the text, don't forget to push the button 'add'. The dialog box will dispappear, but if you would reopen the dialog box you get something like: Do the same for your next chapters. - select your chapter headings - do insert > bookmark - click 'add' Once you've bookmarked all your headings you can make your hyperlinks. Select the text which will be your hyperlink. Click insert > hyperlink (or CTRL+K) In the dialog box, select 'place in this document' In the right pane: scroll down till you see 'bookmarks'. (don't use the ones that are created by the styles on top). Select the bookmark that corresponds with your hyperlink. Word is probably going to overwrite the name you gave to your hyperlink (top of the dialogue box). Overtype the text with what you want it to display. Click OK
