I was wondering if anyone knows whether specifying a secondary category only works on the Smashwords website or does it also effect other retailer's websites? And if so, how?

You'd have to ask Smashwords for the proper answer, but I'd hazard a guess that the secondary category may have something to do with some library systems - for catalog management purposes - or the Library of Congress Classification system.
I don't know if online retailers use that information.
