I have recently published an essay book and need to include Footnotes on it... Though I am following the instruction of the Smashword Style Guide To prevent your NCX from picking up these elements, you can neuter the bookmark by starting the name of your bookmarks with “ref_†So, for example, let’s say your have several endnote references, and they link to endnote bookmarks named 1, 2, 3, 4, etc. By renaming the endnote bookmarks to ref_1, ref_2, Anyway I have tried several times to do this process, Reference Numbers still show up on the TOC list! Any cool tip for this issue....? Thanx a lot!

The style guide warns about having too many interlinked items in a work, so do all your endnote bookmarks appear or just some of them? Also, does the problem exist in PDF, ePUb, and mobi, or just one or two?
I haven't had that problem myself, but you could try changing ref_1 to Ref_1, etc. I name all my bookmarks with a capital letter first and haven't had any major problems.
It could be that you have to nuke your whole book and start afresh if MSWord has added a bunch of hidden crap that is messing up your book formatting.

I started having this same problem in August. My four previous books are formatted exactly the same way and had no problems. I have tried ref_ and Ref_ but it makes no difference. I sent and email to Smashwords and got a reply with some instructions about removing hidden macros. But that did not help. Were you able to find a way to solve the problem?
