I've been trying to upload my ms. to Smashwords all day. In both the epub and mobi versions, a line occurs at the beginning, "Gone to Flowers 215." This isn't in the .doc version, and nothing else is wrong with the formatted text in epub and mobi. Can anyone tell me what's wrong?

Upload your work again. It would appear to be a computer error. I checked Smashwords for "Gone to Flowers 215" and found it is from another author who published a work today.

I just reuploaded the ms. and the line continues to occur in mobi and epub.

I just remembered something that may help. I actually and accidentally uploaded the .doc twice. I deleted one, following instructions to archive it. Maybe it's lurking around somewhere and gumming up the works. I'm wondering if I should just unpublish and start over again, but I'll wait to hear from you.

Unpublish and see if that works. If not, contact Smashwords using their Support system and provide details of your problem.
