Hello Everyone i am facing two major problems while uploading my book on smashwords
I wrote my book in MS WORD 2013 , and then converted it to PDF using Nuance PDF converter, after that i used calibre software to convert it in epub.
Now after uploading it on on smashwords , i am receiving these two errors that i am unable to understand, please help me
1) element "b" from namespace "http://www.w3.org/1999/xhtml" not allowed in this context
2) required attributes missing (repeats 3 times)

The use of Nuance and Calibre to produce an ePub from a MSWord 2013 file may have introduced far too much garbage code or, more likely, really messed up everything.
MSWord produces its own PDF if you absolutely have to have a PDF file, so you don't need Nuance.
Calibre will add MSWord .docx files to your library and allow you to convert them to ePub, so there is no need to use Nuance to produce a PDF prior to conversion to ePub.
For the MSWord .docx files I've tested converting to ePub using Calibre I've found the ToC converted perfectly (all 320 ToC links) as well as all other internal/external links, and the original formatting was retained. Granted, my test files were puzzle files of about 8MB and contained only a couple of graphic images, but everything came out fine after conversion.
Note: prior to uploading an ePub to Smashwords you should run it through the ePub Validator as suggested in the Style Guide as well as elsewhere on this forum.
If your conversion passes the Validator but you get an error message from Smashwords then you can notify Smashwords of your file passing Validator (copy/paste Validator text to your message to Smashwords) and they 'should' allow your work Premium Distribution status.
If you get error messages from Validator then copy/paste those to a new thread in the Epub General Discussion forum section.

I'm with Ted on this one. There seems to be a lot of effort going into creating multiple formats, all of which can create some havoc with closing. Apologies for not being able to offer anything more concrete: it is difficult sometimes to decode these error messages sometimes without seeing the text....

Thank you for your precise reply , it has solved my issue , just wanted to ask one more thing , do i need to create TOC myself in the word document or let the converter do its job? I am a little confuse for it.

I always create my own ToC, then when finished creating the ToC and hyperlinks/bookmarks I use Calibre to create the ePub which I send to the Validator for processing and error report.
I manually create my ToC because I've been working with computers since the early 1970's and have come to appreciate their fondness for bugs.
